- Nebraska Health Information Initiative (NEHII), Nebraska’s statewide health information exchange (HIE), announced it is implementing behavioral health data, such as social determinants of health (SDOH), into its HIE.

The statewide HIE has partnered with Unite Us, a network of social services organizations, to create Unite Nebraska, that aims to connect health and social care providers to enhance care coordination and delivery across the state.

With this partnership, Nebraska citizens can access nutrition services, employment and benefits, and housing through community-based organizations. The two groups aim to break down existing barriers between clinical and social care providers, enabling them to provide care with trackable outcomes data.

“NEHII is delighted to back a statewide infrastructure that will enable better support to address risk areas that influence health outcomes for the people of Nebraska,” said Jaime Bland, CEO of NEHII. 

“The SDoH system we are building with Unite Us will enable the improvement of population-level health outcomes by creating a resource and referral system to effectively address social determinants such as safe housing, access to food, community-based resources, and more. This is one more value-added service available to clinicians and service providers in our state through the NEHII population health utility.” 

NEHII, which connects over 70 percent of providers in the state, has exchanged patient data across the state between health facilities, pharmacists, urgent care providers, and emergency rooms.

Eighty percent of an individual’s health goes beyond clinical care and is impacted by SDOH, the HIE said citing a common statistic.

Research shows that identifying and implementing a patient’s SDOH into the EHR is crucial to finding answers to a state’s largest issues. Relevant SDOH data might include: tobacco and alcohol use, exercise, access to nutritional food, stable housing, reliability of transportation, and many more.

With Nebraska being a rural state, SDOH data is especially critical analyze and report on outcomes for both the healthcare system and the community.

NEHII will utilize the Unite Us platform to integrate existing technology that provides outcomes data, accountability, and transparency.

Together, Unite Nebraska aims to have one SDOH dashboard for all providers across the state, integration into the NEHII platform, and an emphasis on employment and volunteering.

“There's never been a more imperative time to build statewide infrastructure than right now, during the COVID-19 pandemic,” said Taylor Justice, president of Unite Us. “This partnership will enable us to help Nebraskans as they adjust to the social and economic impact of the virus.”

“It's our honor and privilege to work with NEHII, who understands well the need for one holistic and integrated system to improve Nebraskans' health across the state,” Justice concluded. “We are excited to provide the state with their first golden record on individual Nebraskan needs by combining clinical and social care data in one secure location.”

This is another step the statewide HIE has taken to increase the value of its HIE.

In March, NEHII announced it was partnering with NextGate’s cloud-based enterprise master patient index (EMPI) solutions to enhance patient matching.

This EMPI covers over 4 million patient records in the Nebraska region and it will integrate this information from over 300 participating health systems.

“We are proud to be working with NextGate to ensure our members have meaningful data insights at the point of care to treat patients uniquely across the state,” said Bland at the time of the announcement. “Effective care management and collaboration requires fluid data exchange and a holistic view of individuals to make better decisions, understand our populations and deliver the high-quality level of care every resident deserves.”

The implementation of this tool allows for instant interoperability and patient data matching for health systems that are under the NEHII umbrella, which ultimately aims to improve care coordination. It also aims to fix fragmented and diverse EHR applications.
